PREFACE TO FIRST EDITION
Working in the field of Pediatric Cardiology and Intervention was never tiring and boring subject for me rather it has become a passion for me to think about new ideas and to explore it with all the logistic support I have in my hand. Sometimes I find my path a bit hectic and choppy, but with the kind help of Almighty Allah I could overcome all these troubles.
My aim is to give my all research works a shape and to share my experiences with expert in this field, to help newcomers and to keep all research work together for future reference.
Almighty Allah has given me the opportunity to start most of the Pediatric Cardiac Intervention in Bangladesh like first ever Balloon atrial septostomy and PDA coil occlusion in 1998, Pulmonary vulvoplasty in 1999, ASD device closure in 2000, Muscular VSD device closure in 2005, PM VSD device closure in 2006, PDA and RVOT stenting in 2006, coronary fistula occlusion in 2007, coarctation stenting in 2009, PDA closure in newborn with ADO II in 2011. I also got the privilege to start first ever percutaneous pulmonary valve implantation (PPVI) in South Asia on 25th December 2012. 2nd PPVI program was successfully completed on 1st January 2014 without the help of any proctor/ foreign assistance. Our last PPVI program was successfully completed on 10th October 2016.
I always try my best to find a way to help my poor patient and in 2014, I could manage to establish a contact with Little Heart Charity, KSA and Al-Muntada Aid UK for conducting free treatment camp. Our first charity mission was successfully completed in Oct 2014 where total 19 patients underwent cardiac surgery and 21 underwent interventions.Then subsequently we completed 2nd and 3rd mission in Feb and Dec 2015 respectively where almost 200 patients got treatment free of cost (worth 8 crore taka). In every case, financial support was given by Al-Muntada Aid, UK and infrastructural support was given by CMH Dhaka. Since 2016 Al-Muhtada UK is providing support for one hundred cardiac surgeries in Bangladesh. I feel happy to supervise this program under support of Ibrahim Cardiac Center. So far about 180 cases were treated. In February 2018, Qatar Red Cresent Charity and Wadud Moimunnessa Foundation treated 72 cases under my supervision in CMH Dhaka. Implants were given to children free of cost. Hospital services were provided by Bangladesh Army.
I personally raised a trust in the name of my parents (WADUD-MAIMUUNESSA FOUNDATION) with support of my siblings to help poor and needy people specially those suffering from various diseases. It conduct free Friday clinic every week and help distressed people in any kind of disaster. This foundation also award pediatricians once in every two years with gold medal and prize money for their glorious performances in the field of research and selected by Bangladesh paediatric association (BPA) in their biennial seminar.
With the blessing and guidance of respected national professor M R Khan I took the initiative to form a trust for Pediatric cardiac patient in 2014 named ‘Child Heart Trust, Bangladesh’ which is composed of many renowned pediatric cardiologists and pediatric cardiac surgeons of Bangladesh. This trust provides monitoring support to children with congenital heart disease.
I have tried to share all the experiences I gained from my above works. I found so many well-wishers beside me in achieving my goal like my senior teachers, DGMS and Consultant Physician General of Bangladesh Armed Forces, Commandant and Chief Physician of CMH Dhaka, all the members of my department where I work, my other fellow colleagues and my students.
Foremost of all, I gratefully acknowledge those closest to me –my families, my life partner and, of course, my two lovely daughters. I owe them much for their patience, guidance, camaraderie, and, most of all, their love.
PREFACE TO SECOND EDITION
First Edition of “Notes on Basic Pediatric Cardiology” was published two years ago in a little hurry. My plan was to make a book available in our country within short time for the FCPS and MD cardi-ology students. After my plan, that book was to finish within a very short time because of some unavoidable reasons. Moreover many spelling and printing mistakes were detected. More importantly, some students suggested adding pictures of hardware used for interventions and diagnostic catheterizations in pediatric cardiac cath lab. I hope this book will provide all basic information now to students about pediatric cardiac care including neonatal care, intensive care, echocardiography, and cardiac catheterization.
I would like to remind with the respect that most of the contents of this book is very much fundamental. My intention was to share my experience with everyone related with this subject.
As the subject Pediatric Cardiology is still a long way behind to get it’s required shape and form in this sub-continent, I had to face so many challenges since the beginning of my career. So many situations I had to face when there was nobody around me to discuss, or no infrastructure for me to practice what I was trained. The situation was really tough to express.
It was always in my mind that I would never let my students face same problems, rather I would try with my best effort to help and make new generation pediatric cardiologist. In this regard, my initiative was started and I started my writing.
As I mentioned that the first edition of this book was in a bit hurry, I have tried to make this edition easier to understand. I tried to omit using complex terminology and rare diseases, added important topics and figures.
The success of any writing depends on the readers if their purpose is met. I will feel pleased if anybody wants to contribute by sending feedback and suggestions from his own point of view to me through mail or voice.
